Curbside Pickup Service Request Form

While we are taking item requests by phone at (585) 784-5300 during open hours, you can opt instead to fill out this request form at any time.
 
As some staff are working on calls and others are working on email, we ask that you either call OR email, not both, so staff are not duplicating efforts. Our staff will print out your list and will call you to schedule a pickup time once your items are ready. Items will then be placed in our vestibule just before your scheduled pickup.

**If you have been notified that a hold is available and you need to schedule a pickup time, please do not fill out this form. You should call the library during our open hours.

SPECIFIC TITLES YOU WISH TO PICK UP

***LIMIT OF 10 ITEMS PER CARD

(If you are looking for a Thematic Book & Other Format Bundles for kids, fill out a form here.)

For each item, please include the following:
1. Title
2. Format (examples: book, large print book, audiobook, DVD, BluRay, CD, etc.)
3. The author or performer
4. If an item is a specific version, include either the version or the publication year.
